Health Insurance ID (Kitambulisho cha Bima ya Afya)
Passport (Pasi ya kusafiriya)
Tax payer Identification number - TIN (Namba ya Utambulisho ya Mlipa Kodi - TIN)
Zanzibar Resident ID (Kitambulisho cha Mzanzibari Mkaazi)
Marriage certificate and Membership card of social security fund (Cheti cha ndoa na Kitambulisho cha Uanachama wa Mfuko wa Hifadhi ya Jamii)
